I am afraid, it may not be possible for your to effect any change in the birth certificate, at this belated stage. What you may do is to get the changed name notified in the official Gazette, for a prescribed fees. first you publish this matter into one news paper then depands on that you will apply this to corporation of chennai
A notary can do it for you after publishing a notification in a news paper.
but y
you have to make an affidavit in the court in the jurisdiction you are living,infront of a 1st class magistrate. then you have to obtain a form from the corporation where you have been staying .and apply alongwith the affidavit for change of name.,and addressand there should be a declaration in the newspaper.you can easily do it through an advocate within 15 to 20 days with alittle extra.
Changing address is not a big deal. A letter to the relevant authorities is enough. Changing name is the big deal.
You might have to consult a lawer for changing the name officially especially if you are doing business.
