You can do it yourself here are the Canadian and US government information on what you must do. It will require a notary or lawyer at some point but the majority of the work you can do.

In Georgia, I did this as well and simply contacted my ';Secretary of State'; office in Atlanta. They provided me with the forms found on their website.
